Nutrition Program Helps Patients Challenged by Food Insecurity
Funded by a $25,000 grant from the Susan Terry Foundation, CAN Community Healthโs Nourish to Flourish program provides $50 grocery gift cards from partner retailers to patients experiencing unmet basic needs.
The program is designed to help fill gaps where community food assistance resources or external funding are unavailable, giving patients immediate support so they can stay engaged with their care.
